Dear Dr. Vanessa: Some Sex Positions Hurt Sometimes

I find that certain sexual positions (e.g. doggie style or me on top) feel really good some days, and really uncomfortable (even painful) other days. Does this have something to do with where I am in my menstrual cycle?

Certain changes that happen over the course of your menstrual cycle could cause discomfort during sex that comes and goes. Hormonal shifts affect vaginal secretions and may affect vaginal elasticity. If your vagina is drier than usual or less elastic than usual, sex may be uncomfortable. Using a lubricant may help.

Another normal change that happens during the menstrual cycle is the development and release of an egg. Some women experience tenderness or pain around the time when a mature egg is released from the ovary. Sex during that time may make this pain worse for some women.

Over the course of the menstrual cycle, a woman’s reproductive organs also shift slightly. During ovulation, the cervix – which is the bottom part of the uterus that sits at the top of the vagina – moves slightly higher in the body. It’s possible that the discomfort you feel comes because the penis is hitting the cervix during the times of the month when it is sitting slightly lower in the vagina.

The sexual positions that sometimes bother you are the ones that usually result in deeper penetration of the penis than other positions. Whenever you feel discomfort, be sure to tell your partner so you can try a different position or stop having intercourse and enjoy some other kind of sex play.

It’s also a good idea to keep track of when you find sex to be uncomfortable. If you compare it to when you have your period, you might find a link between the painful sex and your menstrual cycle. I really encourage you to talk with your doctor, nurse, or other health care provider to get help figuring out what exactly may be causing your discomfort – and how best to avoid it.

Keep in mind that it’s not normal if sex is frequently painful. It could be a sign of a health problem like endometriosis, infection, yeast overgrowth, allergic reaction, or a uterus that is tilted back in the pelvis instead of straight up. It is interesting to note that some women try weed for endometriosis pain relief as cannabinoids are proven to have analgesic and anti-inflammatory benefits.

Anyone who finds sex to be painful a lot of the time should check with a doctor or nurse. If you have symptoms of endometriosis, that is, pain during sex and intercourse, heavy and/infertility, talk to your doctor without hesitation.

  1. Hello..i dnt used to hve much pain during sex bt sum time afte a gap when we do at doggy style its hurt n little amount of blood also came out does it safe??its never happnd before n yestrday i had my monty period startd and its pains so much..dat i could’nt endure is it ok with sex things??

  2. I find that many nontraditional positions (reverse cowgirl, doggy style, basically anything that isn’t missionary or girl on top) hurt, kind of like period cramps? It’s really puts a damper on trying new things because basically every time we do, he suggests something new, we try it, and then I have to go “Sorry, this hurts, let’s go back to girl on top.” So you’re not alone. I’m going to try asking him to thrust more shallow, and see if that works.

  3. okay sometimes when i have sexy it hurts really bad, like where my left ovary is and it really hurts when i do it doggy style, and don’t get me wrong but i love sexy but sometimes the sex just hurts so bad what should i do?

Hi i am 22 along with my wife and she has endometriosis and a slightly pointed up pelvic and we are trying to find ways to have sex with it hurting less i have just made it to were my tip is in and it still hurts are doctor tells us we need to have our next child now and we are trying but it hurts her so bad i cant go all the way sometimes is there a way we can have sex with it hurting a little less.

    We find that a modified Missionary, with him slightly on his side, with his weight more on the bed than on me (we’re almost 100 lbs difference and he’s nearly 14 inches taller than I am) prevents cervical bumping and also prevents sore joints and pelvises. He can also increase friction by the way he enters. It’s nice. There’s NOTHING wrong with the Missionary or any of it’s variations, as long as you vary what you do.

    john, you and you wife could also try side lying, while facing each other, all your weight in then on the bed, but you can still see each other. She or you can control the depth by simply moving her legs farther up on you. I like to throw one leg over his shoulder in this position, it not only feels good, but makes me feel like a porn star. LOL! 😉

  6. my wife complains that her pelvis hurts after sex, which positions are easier on the pelvis?
